What Services Are Provided?

All services are provided in accordance with NYSDOH regulations and are based on a coordinated plan of care developed by the agency RN after assessing the total needs of the patient and collaborating with the patient’s physician.

AHHCS Services Include:

• Nursing Care – RN and LPN Care.
• Home Health Aides – Remind the patient to take his or her medication and follow physician's order.  Assist patients with personal hygiene, light housekeeping. Follow plan of care assessed by RN.
• Personal Care Services – Homemaking and Housekeeping, Companionship. Follow plan of care assessed by RN.
• Medical Supplies and Equipment (for our clients only).
• Companion Services – Accompanying patients to physician office visits and follow-up clinic visits.
• Case Management Services – Assist Clients and Their Family with Medicaid and Social Security Disability application process, and Nursing home placement.
