What Services Are Provided?

All services are provided in accordance with NYSDOH regulations and are based on a coordinated plan of care developed by the agency RN after assessing the total needs of the patient and collaborating with the patient's physician.

AHHCS Services Include:

• Nursing Care - RN and LPN Care.
• Home Health Aides - Remind the patient to take his or her medication and follow physician's order.  Assist patients with personal hygiene, light housekeeping. Follow plan of care assessed by RN. Accompanying patients to physician office visits, follow-up clinic visits, and family outings.
• Personal Care Services - Homemaking and Housekeeping, Companionship. Follow plan of care assessed by RN. Accompanying patients to physician office visits, follow-up clinic visits, and family outings.
