- To reduce length of hospital stay and readmission by providing continued care and support at home.
- To improve the patient's quality of life by allowing him/her to remain home while receiving individualized care.
- To reduce the negative impact of illness and disability by involving the patient and family in the delivery of Health Care Services at home.
- To maintain and improve patient’s health and help him/her to live as independently and dignified as possible.
- To enable a terminally ill or a dementia patient to remain at home in a familiar surrounding.
- To minimize outpatient clinic visits.
- To offer an alternative to nursing home placement.
Who Is Eligible?
The decision to care for a patient whether he or she is at home, hospital, nursing home and an adult residence is made by the patient, his or her family and the private physician. Patients of all ages are welcome and can request the services during their stay in a hospital or can contact us directly from the community. The AHHCS staff is available to offer a solution to all individuals in need of temporary or long-term home care services.
